That form is there since the tariffs. I ordered an aluminum knob for the drive mode selector from the states a couple of weeks ago and had to fill in the exact same form. I had to pay 35 euro import tariffs for that. That knob was just 20 grams..

I did not but that was pre-tariff days. I thought the seller completed these forms not the buyer.

Nope: tariffs and other import related matters are always handled by the importer (as they are the ones bringing the item into the country and are bound by local laws).

Usually there's a middle man (local subsidiary or reseller) who takes care of this on your behalf but if you're buying directly from a foreign company then you are the importer.

Zunsport grills are available on Amazon, which I'm guessing removes us as the importer, although I haven't tried that yet.

Nope, Amazon US has very lax requirements to list products: https://sellercentral.amazon.com/help/hub/reference/external/G201468460

Given that Zunsport's website has a UK address and their product listing proclaim that they're made in the UK, I'm guessing they're shipping directly from the UK, which makes you the importer.

It was a pretty viable model before the "de minimus" rule was removed and tariffs slapped on a bunch of things.
